What this test is

The anomaly scan (also called Level 2 or TIFFA) is the most detailed scan of pregnancy. Done at 18–22 weeks, it systematically examines the baby's brain, spine, heart, abdomen, kidneys, limbs, and amniotic fluid for any structural anomaly. It also measures growth and locates the placenta. The scan takes 30–45 minutes — longer than other scans because of the systematic checklist.

When you need this test

  • 18–22 weeks of pregnancy (the standard window)
  • First-trimester NT or blood screening raised risk
  • Family history of congenital anomalies
  • Maternal diabetes or other risk factors

How to prepare

  • No fasting required.
  • Drink 2 glasses of water 1 hour before. Avoid being too full.
  • Eat normally — a recently-fed baby is more active and easier to image.
  • Bring all previous antenatal reports.
  • Partner is welcome.

    We follow a checklist of about 30 anatomical structures. Each takes time, especially if baby is in an unhelpful position.
  • Can you tell the baby's gender?
    By law in India, we cannot disclose the gender of the foetus. Please do not ask.
  • What if something is found?
    We refer you to a foetal medicine specialist immediately. Many findings are minor and resolve; serious ones can be planned for.
  • Is this scan compulsory?
    It is strongly recommended in every pregnancy. Many anomalies are silent until birth — early detection allows planning.
