What this test is

PSA is a protein made by the prostate gland. Levels rise with age, prostate enlargement (BPH), infection (prostatitis), and prostate cancer. Most physicians offer annual PSA screening to men from age 50, or from 45 if there is a family history of prostate cancer. Mild elevations are common and usually benign — but persistent rising values warrant further evaluation.

When you need this test

  • Annual screening for men over 50 (over 45 with family history)
  • Difficulty urinating or weak stream
  • Frequent night-time urination
  • Family history of prostate cancer

How to prepare

  • No fasting required.
  • Avoid ejaculation for 48 hours before the test — it can transiently raise PSA.
  • Avoid cycling or heavy exercise for 48 hours before.
  • If you have had a recent urinary catheter, cystoscopy, or prostate examination, postpone the test by 1 week.
  • Mention any urinary symptoms or current antibiotics.

FAQs

  • What is a normal PSA?
Generally under 4 ng/mL, but age-adjusted ranges are preferred. Discuss your specific result with a urologist.
  • Does high PSA mean cancer?
Not necessarily. Most elevations are due to benign prostate enlargement or infection. Persistently rising values warrant ultrasound and possibly biopsy.
  • How often should I test?
Annually from age 50 (or 45 with family history). More often if values are borderline.
